Summary	Form builder: populate from DB the visible value can't be the same as the key
Description	The column used as value (Key) can't be the same as the column used as visible value, this makes the dropdown empty.

I closed this ticket because I think I inadvertently fixed this problem with another bug fix. Let me know if you still have the issue.
